- Job Description
Directs and secures all assessment information on clients for community-based health services targeted at maintaining emergency room clients in their homes as an alternative to frequent emergency room visits or one day hospitalization visits. Summarizes client information and collaborates with members of an assessment team which makes a determination of program eligibility and develops a client care plan.
- Job Requirements
Education: Prefer two years of college in health-related major
License: Prefer LPN Georgia
Experience: Prefer five years’ experience in a medical setting to provide a thorough knowledge of managed care principles, available community resources, and alternative health care delivery systems.
